How did he do that?

Riding home tonight saw a very funny crash but can't work out what happened.

Bloke in front of me pulled away from the lights and somehow managed to flip his front wheel under himself and face plant landing on the floor in a lump, still clipped in, with the bike in the air.

It was a gradual pull away so he didn't lift the wheel while pulling one side of the bars, seen that before and also very funny. All I can think if he managed to catch a loop on his brake lever without knowing and it whipped the wheel round.

Whatever he managed to do it came as a surprise to him and made me laugh.
